Your First Visit

All adults are seen on a private basis. The charge for the first visit is usually the cost of an examination plus 2 - 3 small x-rays.

During your first visit you will have the opportunity to discuss any problems you may be having, or any concerns you may have about the appearance of your teeth. You will be requested to complete a brief medical history questionnaire in advance of your appointment, therefore please bring a list of any medications you are taking.

Following a thorough examination, we will advise you on the full range of treatment options available so that an appropriate written treatment plan can be formulated and provided to you, detailing fees, at the end of your first visit.

 

Denplan

Denplan membership is available to our patients here at Forestmead. A convenient monthly payment covers the cost of all necessary dental treatment provided at the practice (excluding laboratory bills). Worldwide trauma and emergency insurance are also included. Once you are dentally fit, you may be eligible to join Denplan. Should you wish to take up this option, please ask your dentist. Your monthly fee category is then determined by your exisiting dental condition.

 

Children

You are welcome to register your children at the practice. From an early age it is important for them to have regular check-ups to maintain healthy teeth and gums.

Please ask your dentist about registration for children.

 

Registration Period

The registration period at the practice is 2 years. If you do not attend for a routine dental check-up, your registration at the practice will lapse, and you will be required to re-register. We will try to book you with the same dentist, but this may not always be possible.

Improve your smile quickly with the Inman Aligner

A new service is now available at Forestmead Dental Practice. As time goes by your teeth tend to shift, sometimes causing your front top and bottom teeth to become crooked. However with the Inman Aligner, your front teeth can be gently guided to an ideal position quickly and effectively.

Dental Implants

Dental implants are one way of replacing missing teeth. A dental implant is a small titanium screw surgically placed in the jaw providing an anchor for the new tooth or teeth.

Many people choose them because the result is a very natural appearance, they are not removable, you cannot see the supporting mechanism and they are very stable. As they do not require any reduction of your remaining teeth, they are the gold standard technique for replacement. This is a safe and effective way of brightening teeth and removing years of stain build up by coffee, tea, smoking and red wine.

 

Late Night Appointments

Late night appointments are available at Forestmead on Monday and Tuesday evenings until 6.30pm. Call our receptionists for further detail.

 

Commuter Clinic

Early morning Commuter Clinic now available with Mr Dudley for treatment appointments starting at 7.30 on Wednesdays and Fridays.

 

How we use and protect your information

We shall collect, hold and process personal data in accordance with the provisions of the Data Protection Act 1998. These provisions apply to personal data held on a patient’s personal file or on any associated or computerised record.

Where data is held under the provisions of the Data Protection Act 1998, we will ensure that personal data is:

  • You can access any information we store about you by appointment, please contact the Practice Manager to organise a convenient time for this

Security

We will ensure that appropriate measures are adopted to guard against unauthorised and unlawful processing, or the accidental loss, destruction of or damage to data.

Assistance

The subject of data protection is a complicated one. If you require guidance or assistance you should contact our Data Protection Officer Gillian Willett who will be pleased to help you and answer any queries that you may have.

Transfer of information

The only way we would use or transfer your information is for referral purposes to another clinic or hospital. This would only take place after the reasons for your referral have been fully explained to you, you are in full agreement with the referral and you understand how the referral will occur.
